Chief complaints and duration.

A 48 year old female patient who is a housewife by occupation came to the opd with the chief complains of bilateral pedal edema and puffiness of the face.

History of present illness.

Patient was apparently asymptomatic one year back when she developed bilateral pedal edema (pitting type) extending till the knee, knee pain. She was relieved by medication. Three months back the symptoms reappeared where she was again relieved by medication.15 days back the symptoms reappeared and came to our hospital.

History of past illness.

H/O Hypertension since 3 years (on regular medication )

                                                Tab Nicardia 20 mg POBD

K/C/O hypothyroidism since one year (on medication)

                                               Tab Thyroxine 50 mcg POOD

 N/ K/C/O  DM, TB, Asthma and Epilepsy

                                                       

Personal history.

Mixed diet 

Appetite normal

Bowel movements regular 

Burning micturition when on medication.

No addictions


Family history.

No significant family history.

PHYSICAL EXAMINATION.

A. General Examination 

Pallor is present.

Icterus is absent

No cyanosis

No clubbing of fingers

No lymphadenopathy

No malnutrition 

No clubbing of fingers

No oedema of feet and hands.

SYSTEMIC EXAMINATION

B. Cardiovascular system 

No thrills 

No cardiac murmurs

Cardiac sounds: S1 and S2

C. Respiratory system

dyspnea absent 

No wheezing

No Vesicular breath sounds .

Position of trachea - central

D. Abdomen

Abdomen is scaphoid

No tenderness

No Palpable mass 

Bowel sounds anew present 

No bruits

No free fluids

E. Central nervous system

The patient was conscious coherent and Cooperative.
